Most people who come in for neck pain or shoulder pain don’t think their upper back has anything to do with it. But in most cases, it does. The thoracic spine sits between those two areas, and when it stops moving the way it should, both regions above and below it pay the price.

What Is the Thoracic Spine?

Your thoracic spine is the 12 vertebrae that make up your mid and upper back, from roughly the base of your neck down to the bottom of your ribcage. It’s naturally less mobile than the cervical spine above it or the lumbar spine below it, but it still needs to flex, extend, and rotate to function correctly.

When thoracic mobility breaks down, often because of prolonged sitting, previous injury, or years of compensatory patterns, the body finds a way to get the movement it needs. That usually means more demand on the neck and lower back, both of which are not designed to compensate for thoracic stiffness over the long term.

How Thoracic Stiffness Drives Problems Elsewhere

The relationship between thoracic mobility and shoulder function is particularly direct. Your shoulder blade sits on the rib cage, and the way it moves depends heavily on the thoracic spine beneath it. If the thoracic spine is stiff and rounded forward, the scapula tips forward too, which changes the angle of the shoulder joint and creates impingement, rotator cuff overload, and restricted overhead range of motion.

For the neck, a stiff thoracic spine means the cervical spine has to pick up rotational and extension demands it wasn’t meant to handle. Over time, this concentrates stress at the base of the skull and the lower cervical joints, producing the exact pattern of stiffness, tension headaches, and restricted rotation that most tech neck sufferers experience.
